Care Coordination for Personal Care –

Care Coordination is available for all our Silver Sitters Clients. Our experienced Care Managers will assist in streamlining care including:

• Scheduling required appointments with care providers
• Regular Quality Assurance
• Examine health improvement and medication review
• Incontinence ordering and supply management
• Assistance with emergency management planning
