Silver Sitters End-of-Life Care Services

Making each moment comfortable at home –

• As your loved senior enters the journey of the last stage of life, their needs change. This impacts their physical comfort, daily needs, and emotional and spiritual needs. Caregivers at Silver Sitters help you and your loved senior with prominent end-of-life care – intended to optimize his or her quality of life and support the family.
• End of life care is an emotional stage for families when their loved ones may not be able to talk, sit, walk, eat, or make sense of the world. Silver Sitters provides specially trained caregivers who are experienced to provide compassion, moral support, and help around the home during especially emotional situations.

Care Coordination for End-of-Life Care –

Care Coordination is available for all our Silver Sitters Clients. Our experienced Care Managers will assist in streamlining care including:

• We assist in crafting a Client Care Request form that helps us understand the care needs of our client and enable us to support them with the required care.
• We appoint a skilled caregiver who can support seniors physically as well as emotionally.
• Incontinence ordering and supply management.
• Helping with transitions of care including coordinating with hospice crisis care teams.
• Assistance with routine activities, including bathing, feeding, and dressing
