• The approach is everything with a client who has Alzheimer’s & Dementia. The caregivers at Silver Sitters have the training to use different practices and de-escalate possible behavioral expressions caused by the client’s frustration because of the disease.
• The care manager will ensure safety measures are in place for the client. 1:1 care provides engagement and purposeful activities tailored to the individual’s enjoyment.

Care Coordination for Alzheimer’s Care and Dementia Care –

Care Coordination is available for all our Silver Sitters Clients. Our experienced Care Managers will assist in streamlining care including:

• Creating a proactive Client Care Request Form.
• Supporting client’s self-management goals.
• Analyzing and follow-up, including responding to changes in client’s needs.
• Communicating/sharing knowledge.
• Ensuring the client has regular consults with healthcare providers
• Helping with transitions of care.
